NEXT STEPS

Before we can complete your order and send out your collection kit, we need a few more details from you.

  • Please sign into your online account on the Biovault Family website using the password you chose in the sign up process.  You can access your account login here.
  • Complete the customer agreement, consent and information forms that are required in order for us to process your order. We recommend that you complete these as soon as possible, as they are essential in finalising your order.
  • When you have completed the necessary forms, a member of our customer care team at Biovault Family will be in touch and will confirm that your collection kit is in the post.
